Self-certify Evaluation Form
Allow patients to self-certify from a list of acceptable treatments from our conditions database
After the release of RLF-5622, admins can now store acceptable criteria for a condition in our database.
The next goal is to surface this data in the Evaluation form to allow patients to select what treatments they have had, making self-certification easier.
